Please include the full name of the person you would like acknowledged, as well as the name and complete address of where you would like the acknowledgement sent.

These are pre-printed cards with spaces left blank for you to fill in with the individuals name for the appropriate occassion. Please include the quantity of cards you would like, your name and address, and a check payable to THE JUDI SPIVACK MEMORIAL FOUNDATION. Requests should be mailed to the address listed above.
